Atherosclerotic Lesion Size and Vulnerability Are Determined by Patterns of Fluid Shear Stress

چکیده

Background—Atherosclerotic lesions are predominantly observed in curved arteries and near side branches, where low or oscillatory shear stress patterns occur, suggesting a causal connection. However, the effect of shear stress on plaque vulnerability is unknown because the lack of an appropriate in vivo model precludes cause-effect studies. Methods and Results—We developed a perivascular shear stress modifier that induces regions of lowered, increased, and lowered/oscillatory (ie, with vortices) shear stresses in mouse carotid arteries and studied plaque formation and composition. Atherosclerotic lesions developed invariably in the regions with lowered shear stress or vortices, whereas the regions of increased shear stress were protected. Lowered shear stress lesions were larger (intima/media, 1.38 0.68 versus 0.22 0.04); contained fewer smooth muscle cells (1.9 1.6% versus 26.3 9.7%), less collagen (15.3 1.0% versus 22.2 1.0%), and more lipids (15.8 0.9% versus 10.2 0.5%); and showed more outward vascular remodeling (214 19% versus 117 9%) than did oscillatory shear stress lesions. Expression of proatherogenic inflammatory mediators and matrix metalloproteinase activity was higher in the lowered shear stress regions. Spontaneous and angiotensin II–induced intraplaque hemorrhages occurred in the lowered shear stress regions only. Conclusions—Lowered shear stress and oscillatory shear stress are both essential conditions in plaque formation. Lowered shear stress induces larger lesions with a vulnerable plaque phenotype, whereas vortices with oscillatory shear stress induce stable lesions. (Circulation. 2006;113:2744-2753.)

Wall shear stress and local plaque development in stenosed carotid arteries of hypercholesterolemic minipigs

BACKGROUND Wall shear stress is thought to play a critical role in the local development of atherosclerotic plaque and to affect plaque vulnerability. However, current models and hypotheses do not fully explain the link between wall shear stress and local plaque development. We aimed to investigate the relation between wall shear stress and local plaque development in surgically induced common ...

Wall shear stress as a stimulus for intra-plaque hemorrhage in carotid atherosclerotic plaque: An MRI-based CFD pilot study

Motivation Cells present in the arterial vessel wall respond to sustained altered values of hemodynamic forces modifying their structure and function at the cellular, molecular, and genetic levels to return these forces to their physiological values. However, under certain circumstances, this response may lead to pathologies, such as atherosclerosis. Recent studies have suggested that endotheli...

Sites of rupture in human atherosclerotic carotid plaques are associated with high structural stresses: an in vivo MRI-based 3D fluid-structure interaction study.

BACKGROUND AND PURPOSE It has been hypothesized that high structural stress in atherosclerotic plaques at critical sites may contribute to plaque disruption. To test that hypothesis, 3D fluid-structure interaction models were constructed based on in vivo MRI data of human atherosclerotic carotid plaques to assess structural stress behaviors of plaques with and without rupture. METHODS In vivo...

Assessment of the sustainability and instability of bank and bed of the Masil-Muchan river (Astaneh city - Markazi province)

Extended abstract 1- Introduction Rivers are one of the dynamic landforms of natural landscapes that are subject to changes due to environmental variables and human interactions during different times and locations. Changes in the river, erosion of the bed and river bank are natural processes of alluvial rivers that destroy the surrounding agricultural land and damage human installations arou...
